Check anything before you rely on it
Deadlines, coverage rules, and regulator contacts change. Verify against your own plan documents, the date printed on your denial letter, and the current source before acting.
Where the templates stand
The templates are starting drafts, not filings. Nobody reviews what you send, and sending one does not create any relationship with anyone.
No warranty, and what that actually means
This site is provided as is. The law in this area has changed repeatedly since 2024 and parts of it are in active litigation, so some of what is here will go out of date between the day it was written and the day you read it. Every page carries a last-updated date and every legal claim carries the date it was last checked, precisely so you can judge that for yourself.
Some pages state plainly that their legal basis has not been verified against the underlying source. Those pages say so at the top. Take that at face value.
Links to other organisations
Links to advocacy groups, regulators, and legal resources are offered because their material is useful. They are not endorsements, this site has no relationship with them, and it has no control over what they publish or how they handle your data.
Translation
Machine translation is convenient and imperfect. A mistranslated deadline or a mistranslated exclusion could cost you a real appeal. Where accuracy matters, read the English original or get a human translator. See the privacy policy for what translation sends to Google.
